If by ITHVNR you mean reading it with a dictionary, then I think that depends on how good your Japanese is in general. OreTsuba's probably one of the harder works to read for a beginner/someone not well-versed in the language. Some of the reasons include:

1) it boasts a large ensemble cast, each character/group with their intricate personalities/manner of speaking. This what makes OreTsuba as good as it is; you need to know the language to appreciate the scope of the differences/intricacies.
=> the general grammar isn't that hard, minus some exceptions. I'd say it's a few grades above most character games.
=> caveat: one voice/group of voices talk in a pretty heavy delinquent(?) accent; I don't know if most native Japanese readers understood what some characters said fully.

2) it's long and primarily slice of life/comedy. This point primarily means that you have to be OK with reading a lot of text that lack a surface-level purpose. The textbox is also thick compared to other games of its genre.

If you mean reading OreTsuba with a machine translator, then that's probably sacrilegious.

< report >I've read it and enjoyed it even with a rather below average understanding of Japanese. If you already have some basic level of understanding then you should be fine, There a lot of different accents and slangs so you'll improve your understanding of them as well.
